To request a printed copy of our provider directory, or please call Customer Service at 800-878-4445, 8 a.m. to 5 p.m. Monday-Friday, Providence Health Plan will mail a hard copy of the provider directory to you within three (5) business days of your request. Providence Health Plan may ask whether your request for a hard copy is a one-time or permanent request. Customer Service can also provide information about practitioner race/ethnicity upon request.
To report information you believe to be listed inaccurately in the provider directory, please call 800-878-4445, send an email to PHPPRcontactus@providence.org or login to your myProvidence account and use the secure chat feature. Please be sure to have the provider(s) and/or facility name, location and description of the error available.

Leadership and Board
The North Carolina Infectious Diseases Society is led by a five member board of directors.
David Weinrib, MD, serves as the president of the North Carolina Infectious Diseases Society. He is currently a practicing physician at Atrium Health - Carolinas Medical Center (Charlotte) and a Clinical Professor at the Wake Forest University School of Medicine.
Jennifer Emberger, MD, serves as the vice president. She is a practicing infectious disease physician and clinical assistant professor at East Carolina University/ECU Health Medical Center.
Secretary
Michael DeWitt, MS, serves as the secretary. He is an infectious disease epidemiologist and ecologist at Wake Forest University School of Medicine.
Treasurer
Julia Cook, MD, serves as the treasurer. She is a practicing infectious disease physician and assistant professor at Wake Forest University School of Medicine.
Assistant Secretary
Jacob Pierce, MD, serves as the assistant secretary. He is an assistant professor of infectious diseases and Medical Director of Infection Prevention for ECU Health system.
Assistant Treasurer
Mike Schwartz, PharmD, serves as the assistant treasurer. He is a clinical professor at East Carolina University College of Allied Health Sciences, Department of PA Studies and he also holds an appointment as Adjunct Professor in the Brody School of Medicine Division of Infectious Diseases.
Note: It is HIGHLY encouraged that non-physician members take an active role in NCIDS leadership and as such may run for ANY position on the board. However, the Society wishes to ensure that it always has representation with a non-physician serving at a minimum in an at-large capacity.
